I had a favicon and when I changed my theme it suddenly disappeared. It is in my FTP files on my host site called FAVICON.ICO
How do I reload it again??? Can I do this in my Wordpress Dashboard?

Anyone who can recommend a plug-in that would create a pop-up window the first time someone visits my blog with a first name, last name and e-mail address field?
This should only happen to a new visitor and they would click the X to close the box if not interested in subscribing to posts.
